I was told a family of 4 would not "fit" into the cat. 12 stateroom. I thought a cat. 12 was for 3 occupants or less. So, I was under the impression we would automatically be "upgraded" to a cat. 11. Is this incorrect?

There are no cat 12's that sleep 4. You get booked into what is called a GTY (guarantee) category. A cat 12 GTY is also called "run of ship"
Run of ship means that they can put you in any cabin that sleeps 4 from cat 12 (which has none) to a Cat 1 (which never happens).
You do not find out your cabin number, or what category you were assigned until close to your sail date.
Cruise Lines do this to fill in spots where they need them.
So you may be upgraded to an 11, 10, 9, etc. Most common is 11.
